Direct Support Professional-Residential – Substitutes
The Direct Support Professional-Residential will provide training and care to consumer in assigned residence; transport consumers as needed; ensure the well-being of assigned consumers; maintain cleanliness in assigned residence. Duties are semi-routine requiring judgments within established agency guideline.
Service Coordination – Case Manager – PT and FT 770 Electric Drive4yr/2yr degree in social work or human service related; at least of one year of experience working with individuals with disabilities. Knowledge of Medicaid / Fee For Service Billing.
LPN - FT
The LPN will provide direct nursing care to assigned residential consumers; monitor the general health of assigned consumers; maintain medical charts and write quarterly progress notes; oversee medication administration; schedule medical appointments and escort consumers when necessary; participate in team meetings; complete nursing assessments; provide first aid and assist with medical emergencies when needed; participate in the nursing on-call rotation. •Completion of an accredited degree program or LPN School of Nursing •Valid current LPN license from the South Carolina State Board of Nursing
Lawn Maintenance Technician FT
Responsible for the general upkeep of the grounds for all agency owned facilities. Facilities are located throughout the Sumter community. It is the expectation of agency management that all grounds and exterior portions of agency property remain in a neat, well maintained and appropriate condition at all times.
High School Diploma/GED and 1 year of lawn maintenance experience.
Community Training Home I (Caregiver)
The Sumter County Board of Disabilities and Special Needs is recruiting qualified caregivers to assume care for individuals with developmental disabilities within their home. This situation resembles closely that of foster care homes, usually associated with children and “host homes” which is a term widely used in other regions of the country. The caregiver would be required to have their personal home licensed per regulations of the South Carolina Department of Disabilities and Special Needs and the South Carolina Fire Marshall’s Office.
he licensed caregiver would be required to provide the necessary supervision to ensure the consumer(s) in their care was safe, protected and always accounted for. The Community Training Home-I also requires a habilitation plan that incorporates personalized care, supervision and individualized training to each consumer served. The maximum number of consumers that can be served in any home is two. The consumer(s) would become a part of the family structure.
